Abstract: THE QUESTIONNAIRE IS NEEDED TO GATHER INFORMATION NECESSARY TO IDENTIF POTENTIALLY QUALIFIED APPLICANTS FOR AN AFROTC SCHOLARSHIP. THIS ONE-PAGE QUESTIONNAIRE WILL ENABLE AIR FORCE TO LOWER THE NUMBER OF SCHOLARSHIP APPLICATION BOOKLETS BEING PRINTED FROM 400,000 TO 50,000 SAVING $66,000. APPROVAL BY FEB 85 MAY PROVIDE DATA FOR GREATER SAVINGS.
