THE MEDICAL INFORMATION COLLECTED ON
THE BLOOD DONOR'S MEDICAL HISTORY IS USED BY THE HOSPITAL MEDICAL
PERSONNEL IN THE BLOOD BANK TO DETERMINE IF A PROSPECTIVE VOLUNTEER
BLOOD DONOR IS FREE FROM ILLNESSE THAT MIGHT CAUSE HARM TO A VA
PATIENT IF TRANSFUSED WITH THE DONOR'S BLOOD.
On behalf of this Federal agency, I certify that
the collection of information encompassed by this request complies

1320.8(b)(3).
