Abstract: SERVICEMAN'S FAMILY PROTECTION PLAN (RSFPP) AND/OR SURVIVOR BENEFIT PLAN (SBP). THE AIR FORCE ACCOUNTING AND FINANCE CENTER (AFAFC), SURVIVOR BENEFIT DIVISION MUST RECEIVE AN APPLICATION FOR SURVIVOR ANNUITY. THE APPLICATION IS NEEDED TO VALIDATE ENETITLEMENT TO A SURVIVOR ANNUITY, ESTABLISH CURRENT STATUS OF BENEFICIARIES OF A SURVIVOR ANNUITY, DETERMINE THE TYPE AND AMOUNT OF ANNUITY TO BE PAID, AND ESTABLISH ANNUITY ACCOUNT.
