Abstract: THIS FORM IS USED BY STATE AGENCIES TO REQUEST THE INTERNAL REVENUE SERVICE (IRS) TO COLLECT DELINQUENT SUPPORT OWED UNDER A COURT OR ADMINISTRATIVE ORDER. THE INFORMATION IS USED BY IRS TO IDENTIFY, LOCATE AND START TAX DELINQUENT ACTION AGAINST AN ABSENT PARENT WHO HA FAILED TO PAY COURT OR ADMINISTRATIVE ORDERED SUPPORT. THE DATA COLLECTED ON THIS FORM IS INCLUDED IN THE REQUIRED ANNUAL REPORT TO CONGRESS.
