Abstract: THIS FORM COLLECTS THE DATA NECESSARY TO DETERMINE STUDENT ELIGIBILITY FOR FEDERAL STUDENT AIR. THE INFORMATION IS USED TO CALCULATE A STUDENT AID INDEX FOR THE DISTRIBUTION OF PELL GRANTS AND A UNIFORM METHODOLOG NUMBER WHICH FINANCIAL AID ADMINISTRATORS MAY USE TO AWARD OTHER TYPES OF FINANCIAL AID.
