Abstract: THIS FORM COLLECTS THE DATA NECESSARY TO DETERMINE STUDENT ELIGIBILITY FOR FEDERAL STUDENT AID WHEN A STUDENT'S FAMILY FINANCIAL SITUATION CHANGE FROM THE USUAL BASE YEAR. A STUDENT AID INDEX FOR THE DISTRIBUTION OF PELL GRANTS AND A UNIFORM METHODOLOGY NUMBER ARE CALCULATED FOR THE USE OF FINANCIAL AID ADMINISTRATORS.
