Abstract: FAMILY SUPPORT CENTERS SERVE AS FOCAL POINTS FOR ASSISTANCE PROGRAMS AND ACTIVITIES THAT HELP AIR FORCE FAMILIES. COLLECTION OF INFORMATION IS NECESSARY TO ASSIST AIR FORCE MEMBERS AND THEIR FAMILIES TO RESOLVE THE SPECIAL PROBLEMS ASSOCIATED WITH MILITARY LIFE. THE FORMS WILL HELP ENSURE THAT THE NEEDS OF PERSONS SEEKING ASSISTANCE ARE MET EXPEDITIOUSLY AND EFFECTIVELY.
