FORM FNS-418, REPORT OF THE SUMMER FOOD SERVICE PROGRAM FOR CHILDREN, PROVIDES INFORMATION ON WHIC TO BASE A PROFILE OF SPONSORS, SITES, AND MEAL TYPES, AND ALSO PROVIDE INFORMATION ON WHICH TO DETERMINE COMMODITY ENTITLEMENTS. FORM FNS-80 AGREEMENT BETWEEN SPONSOR AND USDA, ENSURES THAT THE SPONSOR IS AWARE OF PROGRAM RESPONSIBILITIES.
