Terms of Clearance: APPROVED WITH CONDITIONS. THIS REQUEST IS APPROVED THROUGH OCTOBER 31, 1988. SINCE THERE ARE SEVERAL SIMILAR SBA INDIVIDUAL ASSISTANCE PROGRA THE SBA SHOULD EXAMINE THE FEASIBILITY OF A STANDARD FORM TO EVALUATE THE ASSISTANCE PROVIDED. SBA SHOULD PROVIDE OMB WITH THE RESULTS OF THIS EXAMINATION NOT LATER THAN MAY 1, 1988. IN ADDITION, THE SBA SHOU CONSIDER THE USEFULNESS OF HAVING ONE FORM FOR THIS PROGRAM.
