Abstract: THE INFORMATION COLLECTED BY THIS FORM IS OBTAINED FROM LANDLORDS AND USED TO MAKE RENTAL SUBSIDY DETERMINATIONS IN THE SUPPLEMENTAL SECURITY INCOME (SSI) PROGRAM. THE AFFECTED PUBLI CONSISTS OF LANDLORDS WHO ARE SUSPECTED OF SUBSIDIZING A RENTAL ARRANGEMENT BETWEEN THEMSELVES AND AN SSI RECIPIENT.
