Terms of Clearance: This ICR is not approved. A letter giving the details of this action has been sent to Dan Fiorino of OPPE. The following is a brief summary of the letter: (1) reporting burden should be minimized and then an estimate included in the ICR, (2) the form and its instructions should be revised to eliminate unnecessary information including the supplemental submissions mentioned on the reverse side of the form. The current OMB clearance will be extended for three months to allow OPP to make the necessary changes and submit a revised ICR.
