Abstract: THE INFORMATION COLLECTED BY THE USE OF THE FORM SSA-8740 IS NEEDED TO ENABLE SSA TO REFER RECIPIENTS TO AN APPROPRIATE STATE AGENCY FOR DRUG ADDICTION/ALCOHOLISM TREATMENT. THE AFFECTED PUBLIC IS COMPRISED OF STATE AGENCIES WHO HAVE AGREED TO REFER AND TREAT SSI RECIPIENTS WHO ARE DRUG ADDICTS/ALCOHOLICS.
