Abstract: THE INFORMATION COLLECTED BY THE USE OF THE PILOT TEST FORMS SSA-3368T, SSA-3369-TS AND SSA-3369TL WILL BE USED TO DETERMINE IF THE PROPOSED FORMS ARE EFFECTIVE IN SIMPLIFYING THE CLAIMS-TAKING PROCESS AND WARRANT NATIONAL IMPLEMENTAION.
