Abstract: THE INFORMATION OBTAINED VIA THIS FORM I NEEDED BECAUSE IT IS REQUIRED BY LAW TO ASSIST THE OFFICE OF CHILD SUPPORT ENFORCEMENT IN AUDITS OF STATE AGENCY PROGRAMS, AND IN MONITORING AND EVALUATING STATE CHILD SUPPORT ENFORCEMENT ACTIVITIES.
