Terms of Clearance: This information collection request is approved subject to the following: OHDS has agreed to conduct the study using a phased approach in which the first phase also serves as a pilot test for the methodology, At the end of the first phase OHDS will examine the results to assess problems with response rates, nonresponse bias, viability of controls, feasibility of of parental contact, and the utility of the focus groups, and If on the basis of its assessment, OHDS determines the study design or survey need to be revised, OHDS will resubmit the request.
