Abstract: THIS SURVEY IS PART OF THE PROCESS EVALUATION OF THE DEMONSTRATION. THE SURVEY WILL BE ADMINISTERED TO ALL CASE MANAGERS IN THE DEMONSTRATION PROGRAMS, AND WILL PROVIDE CRITICAL INFORMATION WITH WHICH TO ASSESS THE IMPORTANCE OF THE BACKGROUNDS, EXPERIENCES AND ATTITUDES OF CASE MANAGERS TO THE SUCCESS OF THE INTERVENTION.
