Abstract: THE INFORMATION IS NEEDED TO CLARIFY MISSING OR INCOMPLETE DATA ON THE APPLICATION FOR EMPLOYER IDENTIFICATION NUMBER (SS-4). THE RESPONSES ARE TRANSLATED INTO VARIOUS CODES.
