Abstract: THE INFORMATION COLLECTED BY THE USE OF THIS FORM IS NEEDED IN ORDER T DETERMINE ONE-HALF SUPPORT, WHICH IS A REQUIREMENT FOR ENTITLEMENT TO PARENT'S BENEFITS.
