This information
collection request was submitted pursuant to a reques for all
collections used when importing merchandise into the U.S. These
collections were reviewed as candidates for the Customs' "Paperless
Entry Project". Based on our review, this collection has been
designated for a 1991 time track for incorporation into the
Customs' system. Development plans and a timetable for making
progres toward the paperless system should be submitted to OMB by
August 31, 1989. CDC should notify OMB as early as possible about
any problems it encounters during the planning and development
process.

THIS DATA COLLECTION IS THE
APPLICATION FOR A PERMIT TO IMPORT OR TRANSFER AGENTS OR VECTORS OF
HUMAN DISEASES. THE IMPORTER PROVIDES INFORMATION ON METHODS OF
TRANSPORTATION AND PROPOSED USES OF MATERIAL IMPORTED. IN THE
INTERSTATE SHIPMENT OF THESE VECTORS, CARRIERS MUST REPORT THE
DISCOVERY OF DAMAGED PACKAGES AND THE SENDER MUST REPORT AN LOST
SHIPMENTS.
