THE INFORMATION COLLECTED BY THIS FORM WILL BE USED TO DETERMINE IF WORKERS' COMPENSATION OFFSET IS BEING CORRECTLY APPLIED TO SOCIAL SECURITY BENEFICIARIES AS REQUIRED BY SECTION 224 OF THE SOCIAL SECURITY ACT. THE AFFECTED PUBLIC CONSISTS OF PEOPLE WHO ARE RECEIVING BOTH SOCIAL SECURITY AND WORKERS' COMPENSATION BENEFITS.
