Abstract: THE NOTIFICATION FORM CONTAINS OWNERS AND OPERATOR INFORMATION FOR FACILITIES THAT HANDLE HAZARDOUS WASTE. AN OWNER/OPERATOR MUST SUBMIT THIS FORM TO EPA PRIOR TO HANDLING HAZARDOUS WASTES OR PRIOR TO HANDLING NEW HAZARDOUS WASTE STREAMS. EPA USES THIS INFORMATION TO MAINTAIN AN INVENTORY OF ALL HAZARDOUS WASTE ACTIVITIES THROUGHOUT THE NATION.
