Abstract: FORENSIC DRUG TESTING UNDER EO 12564 AND DHHS GUIDELINES REQUIRES AN UNBROKEN CHAIN OF CUSTODY WITH ABSOLUTE IDENTIFICATION OF A SPECIFIC SPECIMEN AS BELONGING TO A CERTAIN INDIVIDUAL. THE PROPOSED FORM ACCOMPLISHES THIS, GIVES INFORMATION NEEDED TO IDENTIFY THE INDIVIDUAL TRACK THE SAMPLE THROUGH COLLECTION, TRANSPORTATION TESTING, PROVIDE THE RESULTS TO THE MRO, AND ALLOWS FOR MRO VERIFICATION.
