Terms of Clearance: This ICR is approved as revised. Any further changes in the questionnaire resulting from the evaluation of the interim report of nine State programs shall be submitted to OMB for review prior to incorporation in the final instrument. The burden box must be move to the beginning of the questionnaire in the introduction so respondan are informed prior to completing the questionnaire what the expected time requirements are. EPA has done a good job in working with respondants and improving the questionnaire based on their comments.

Abstract: THE STATE QUESTIONNAIRE WILL PROVIDE INFORMATION TO EPA ON THE STATUS AND OPERATIONS OF STATE REVOLVING FUND PROGRAMS. EPA WILL USE THE INFORMATION TO PREPARE A ONE-TIME REPORT TO CONGRESS AS REQUIRED IN SECTION 516(G) OF THE CLEAN WATER ACT AS AMENDED BY THE WATER QUALITY ACT OF 1987.
