Abstract: THIS REPORT IS NECESSARY TO ESTABLISH QUALIFICATIONS OF ELIGIBILITY TO RECEIVE VOLUNTARY PHYSIOLOGICAL TRAINING AND WILL BE USED AS PROPER EVIDENCE OF TRAINING. THE FORM IS FILLED OUT BY PILOTS AND AIR CREWMEMBERS FOR APPLICATION TO RECEIVE VOLUNTARY TRAINING.
