THE INFORMATION COLLECTED BY THIS FORM IS NEEDED TO PROVI AN ADMINISTRATIVE LAW JUDGE (ALJ) WITH AN UPDATED LIST OF MEDICATIONS WHICH ARE USED BY THE CLAIMANT. THIS ALLOWS THE ALJ TO FULLY CONSIDER DISABILITY ASPECTS OF THE CLAIM. THE AFFECTED PUBLIC CONSISTS OF CLAIMANT'S FOR DISABILITY BENEFITS WHO HAVE REQUESTED A HEARING.
