Abstract: THE INFORMATION REQUESTED WILL ASSIST THE DEPARTMENT IN SELECTING APPLICANTS WHO MEET PROGRAM REQUIREMENTS AND DEMONSTRATE THE GREATEST NEED FOR THE MODERATE REHABILITATION SINGLE ROOM OCCUPANCY (SRO) PROGRAM FUNDS.
