THIS IS TO IDENTIFY WHERE LIQUOR WAS SOLD BY THE DRINK AT LOCATIONS
OTHER THAN WHAT WAS LISTED ON THE SPECIAL TAX STAMP ISSUED TO THE
CATERER. THE FORM IS FILED IN DUPLICATE BY THE CATERER, ALONG WITH
AN AMENDED ATF F5630.5.
