Abstract: THE DEPARTMENT OF DEFENSE MEDICAL EXAMINATION REVIEW BOARD (DODMERB) NEEDS THIS FORM TO COLLECT MEDICAL HISTORY OF APPLICANTS TO DETERMINE MEDICAL ACCEPTABILITY FOR ENTRY INTO THE 5 SERVICE ACADEMIES, USUHS, AND THE ROTC PROGRAMS, INCLUDING THE COLLEGE SCHOLARSHIP PROGRAM.
