Abstract: FORM IS USED BY INSTITUTIONS OF HIGHER EDUCATION TO REQUEST APPROVAL O THEIR GRADUATE PROGRAMS OF STUDY SO THAT THEY MAY NOMINATE STUDENTS FO FELLOWSHIP AWARDS. THE STUDENT NOMINATION FORM BECOMES PART OF THE AWARD DOCUMENT AND IS USED BY INSTITUTIONS TO REPORT ANNUALLY ON THE AMOUNT OF FUNDS SPENT PER FELLOWSHIP.
