the HUD "Applicant Disclosure Form" for 15 months. We understand
that the final rule making this form effective will be published in
CY1990. When resubmitted for OMB review, HUD shall assess whether
this form has achieved the desired results, shall discuss any
problems, such as completeness or quality, that may have arisen
with respect to the information reported on this form, and shal
determine whether revisions that reduce the reporting burden are
consistent with the goals of this program.

INFORMATION WHICH WILL INCLUDE OTHER GOVERNMENT ASSISTANCE BEING
REQUESTED, NAMES AND FINANCIAL INTEREST OF ALL INTERESTED PARTIES,
AND A REPORT OF EXPECTED SOURCES AND USES OF FUNDS A $200,000
THRESHOLD APPLIES TO THIS DISCLOSURE REQUIREMENT.
