this survey as revised through July of 1993. However we are
concerned that the new question about the costs of the vaccine,
office visits, and lab testing, as currently drafted may be too
burdensome to respondents. Thus, the agency shall revise the cost
question on Attachment C to emphasize that "approximate" or
"estimated" answers are desired. Such emphasis could be achieved by
adding the word "approx." before the "cost: $" portion of the
question or by adding a note to the question stating that the
respondent is not expected to review any records in answering the
cost questions.
