The justification is inadequate for the following reasons: 1) current response rate data for this survey (required by the SF-83 instructions) has been omitted, 2) characterization of the design as a stratified random sample is inaccurate (the design appears to be a simple "cut-off" sample), 3) neither the proposed follow-up cards nor a telephone follow-up script (or description) were included in the package, 4) the proposed sample size has not been justified, and 5) the actual field sample selection procedures and controls have not been documented.
