Abstract: THE FNS-191 FORM IS USED BY WIC AND CSFP LOCAL AGENCIES TO COLLECT RACIAL/ETHNIC DATA ON EACH PARTICIPANT FOR ONE MONTH OF THE YEAR. THE FNS-101 FORM IS COMPLETED BY FSP AND FDPIR LOCAL AGENCIES ON ALL HOUSEHOLDS FOR ONE MONTH OF THE YEAR. THE COMBINED COLLECTION OF RACIAL/ETHNIC DATA IS USED TO MONITOR THE WIC, CSFP, FSP, AND FDPIR
