Abstract: THIS FORM I REQUIRED UNDER THE DEPARTMENT OF LABOR'S NEGOTIATED MERIT STAFFING PLAN FOR POSITIONS IN THE LOCAL 12 BARGAINING UNIT TO COLLECT INFORMATION BY THE PERSONNEL OFFICE FROM THE APPLICANT'S SUPERVISOR.
