Abstract: THE QUESTIONNAIRE IS DESIGNED TO PROVIDE THE DEPARTMENT WITH CURRENT INFORMATION REGARDING AN AGENCY'S ABILITY TO SATISFACTORILY ADMINISTER ITS LAW OR ORDINANCE AS REQUIRED BY THE REGULATION, PART 115. THE INFORMATION ENABLES OUR REGIONAL FHEO STAFF TO CONDUCT ON-SITE PERFORMANCE ASSESSMENTS OF THE AGENCY.
