AFTER IT IS COMPLETED BY A SENIOR BANK OFFICER, THE QUESTIONNAIRE PROVIDES INFORMATION REGARDING THE BANK'S EFFORT TO SERVE THE CREDIT NEEDS OF ITS LOCAL COMMUNITY.
