Terms of Clearance: This ICR is approved for three years on the condition that the OMB No. (2030-0013) and a burden disclosure statement be placed on Forms 5700-48 and 5700-41. The burden statement shall be for the burden imposed by the form itself (not the total collection), i.e. 4 hours for 5700-48 and 2.5 hours for 5700-41 and include the OMB No. (2030-0013) parenthetically after the words "Paperwork Reduction Project." This ICR no longer covers the burden from State, local or Indian tribal governments.
