Abstract: THE ATTACHED FEMA FORM 95-5 IS USED TO SUBMIT THE ANNUAL PROJECTION AS WELL AS COLLECT QUARTERLY DATA. FEMA FORM 95-5A IS USED BY THE STATES PARTICIPATING IN THE EMERGENCY MANAGEMENT TRAINING (EMT) PROGRAM TO PROJECT ANNUALLY AND REPORT QUARTERLY THE NUMBER OF TRAINING ACTIVITIE PARTICIPANTS, AND COSTS.
