Abstract: HCFA IS REQUESTING APPROVAL OF A PRESCRIPTION FORM WHICH WILL BE COMPLETED BY DEMONSTRATION PROVIDERS WHICH WILL VERIFY THAT THE MEDICARE BENEFICIARY MEETS THE DEMONSTRATIO ELIGIBILITY CRITERIA. THE FORM WILL BE USED BY THE DEMONSTRATION CONTRACTOR TO CHECK MEDICARE ELIGIBILITY, RANDOMIZE APPLICANTS, AND PROVIDE BASELINE INFORMATION FOR THE EVALUATION.
