Abstract: THIS FORM PROVIDES INFORMATION ON THE PROPOSED MERGER, ON THE PRO FORMA FINANCIAL CONDITION OF THE APPLICANT, ON COMPETITIVE ISSUES AND THE PROPOSED ADVANTAGES TO THE PUBLIC'S NEEDS AND CONVENIENCE.
