Abstract: THIS INFORMATION WILL BE USED BY THE DEPARTMENT TO DETERMINE WHETHER A INSTITUTION THAT HAS LOST ITS ELIGIBILITY TO PARTICIPATE IN THE GSL PROGRAMS MAY CONTINUE TO PARTICIPATE.
