IF THE PAYEE WISHES TO CLAIM FORGERY, HE OR SHE ANSWERS QUESTIONS ON THE FORM AND SIGNS AND RETURNS IT TO THE ADJUDICATION DIVISION. CLAIMS EXAMINER REVIEW THE CLAIM TO DETERMINE FINAL ACTION ON THE CASE.
