Approved as
amended by ED's 8/26/91 memorandum to OMB. One-year approval is
granted, which will allow ED to use this form for FY 1992, while
testing its utility so that adjustments can be made in the context
of the Rehabilitation Act reauthorization. ED shall submit both the
RSA-636 and the RSA-911 for PRA review at that time. Last, E shall
ensure that Supported Employment grantees receive the most recen
version of program regulations; as the 7/1/90 version of the 34 CFR
363.7 indicates, these provisions were amended in May 1988.

THE RSA COMMISSIONER MUST COLLECT AND
REPORT INFORMATION FOR EACH INDIVIDUAL WITH SEVERE HANDICAPS IN
SUPPORTED EMPLOYMENT SERVED UNDER TITLE VI. THIS REPORT MUST BE
SUBMITTED TO THE PRESIDENT AND CONGRESS WITHIN 120 DAYS OF EACH
FISCAL YEAR. AFFECTED PUBLIC WILL BE 84 STATE AGENCIES.
