Abstract: THIS FORM IS USED BY ALL HOSPICE FACILITIES APPLYING FOR ENTRANCE INTO THE MEDICARE PROGRAM. THE INFORMATION IS USED BY STATE AGENCIES THAT PERFORM THE MEDICARE CERTIFICATION SURVEY PROCESS TO SCHEDULE ONSITE SURVEYS AND TO ENSURE THAT THE APPLYING FACILITY MEETS PRELIMINARY REQUIREMENTS PRIOR TO RECEIVING AN ONSITE SURVEY.
