approval of this one-time pilot questionnaire only through July
1992. No further extensions of this information collecti will be
granted unless the agency demonstrates that 1) the information
collection burden associated with this questionnaire is justified
by i usefulness in helping to reduce long-term back injuries; and
2) the agency demonstrates that the prospective predictive power of
this questionnaire is sufficient to assist in program
management.

THROUGH TELEPHONE INTERVIEW FROM TREATING PHYSICIANS WHO ARE CARING
