Terms of Clearance: Approved through May 1993. OMB notes that this information collection has been in use without OMB approval. The Bureau of Public Debt has violated the Paperwork Reduction Act (PRA), and implementing regulatio at 5 CFR 1320.4 which state that "An Agency shall not engage in a collection of information without obtaining Office of Management and Budget approval of the collecton of information and displaying a currently valid OMB control number..." Furthermore, "Notwithstanding any other provision of law, no person shall be subject to any penalty for failure to comply with any collection of information: (1) That doe not display a currently vailid OMB control number..." (1320.5). In its next submission for review under the PRA, the Department should report on efforts to consult with persons completing this form about ways in which this collection might be improved. As requested, you may omit printing the expiration date on this form.
