Abstract: THE NATIONAL FIRE ACADEMY EXTENSION SERVICES COURSE EVALUATION FORM IS USED IN ALL EXTENSION SERVICES DELIVERIES OF ACADEMY COURSES. THE FOR IS USED PRIMARILY TO ASSESS THE EFFECTIVENESS OF THE COURSE MATERIALS AND INSTRUCTOR DELIVERY. THE DEMOGRAPHIC INFORMATION IS USED IN DEVELOPING NEEDS ASSESSMENTS AND IDENTIFYING THE STUDENT POPULATION'S REPRESENTATION.
