Abstract: THIS FORM PROVIDES REGISTERED ENGINEERS AND ARCHITECTS A STANDARD MEAN OF CERTIFYING THE FLOODPROOFED CONSTRUCTION OF BASEMENTS LYING BELOW T BASE FLOOD ELEVATION. THE CERTIFICATION IS PROVIDED TO THE FLOOD INSURANCE AGENT AND COMMUNITY BUILDING OFFICIAL AS RECOGNITION THAT TH BASEMENT IS BUILT ACCORDING TO THE STANDARDS OF THE NATIONAL FLOOD INSURANCE PROGRAM. THE HOMEOWNER MUST PAY FOR THE CERTIFICATION.
