Abstract: THIS FORM IS USED WHEN APPLICATION IS MADE BY A FUNERAL HOME FOR THE FUNERAL EXPENSES OF A DECEASED POSTAL DEPOSITOR. THIS FORM IS COMPLET BY A RELATIVE OF THE DECEASED DEPOSITOR CERTIFYING THAT THE BILL SUBMITTED BY THE FUNERAL HOME IS CORRECT. ENTITLEMENT TO THE FUNDS IS BASED ON THIS DATA TO INSURE PROPER PAYMENT.
