Terms of Clearance: In accordance with the Paperwork Reduction Act and 5 CFR 1320, this information collection has been approved for six months. This review shou include a description of the rescinded May 18, 1992, memorandum which changed the instruction for this form, and any further actions taken b the agency since that time.
